Requests for official transcripts must be submitted in writing by the student or alumni. A $10 fee will be assessed for each official transcript. Any request for transcripts or other official documents can be provided by the Registrar’s Office.

Please print the Document Request Form or obtain one from the Registrar’s Office. You must use a black or blue ink pen when completing and signing the form. Return this request form to the Registrar’s Office via one of the following options:
